Welcome to Coolo Care! We are dedicated to ensuring the well-being of your loved ones through our in-person Care Check services. By signing up for our services, you agree to the following terms of service:

1. Scope of Services

1.1. Coolo Care provides Care Check services. This includes visiting your loved ones to assess their:

  • Overall state and environment.
  • Hygiene and cleanliness.
  • Food and water intake.
  • General observations about their physical and emotional condition.
  • Verification of whether they have been visited by their nurse or healthcare provider.

1.2. Coolo Care does not provide medical consultations, diagnoses, or treatments. Any concerns requiring medical attention will be documented and communicated to the responsibleparty for appropriate action.

2. Guardian/Responsible Party Verification

2.1. During the intake meeting, Coolo Care requires verification that the individual signing up for services is the guardian, Power of Attorney (POA), or legally responsible party for the loved one.

2.2. The following documentation must be presented during the intake meeting:

  • Legal proof of guardianship, POA, or responsibility.
  • Valid photo identification.

2.3. Services will not commence without verified documentation.

3. Privacy and Data Storage

3.1. Coolo Care is committed to maintaining the confidentiality of all personal information. All client and patient information will be:

  • Stored in HIPAA-compliant databases.
  • Secured with long-term storage protocols for retrieval as needed.

3.2. Access to data is strictly limited to authorized personnel and will only be shared with the designated responsible party.

4. Visit Documentation

4.1. Coolo Care documents every visit to ensure transparency and quality of service.
Documentation includes:

  • Observations on the loved one’s state, environment, and well-being.
  • Notes on hygiene, food, and water intake.
  • Confirmation of whether the loved one has seen their nurse or healthcare provider.

4.2. Findings will be shared with the responsible party on a weekly or bi-weekly basis, as selected during the intake process.

5. Responsibilities of the Responsible Party

5.1. The responsible party agrees to:

  • Provide accurate and up-to-date information about the loved one.
  • Communicate any changes in care needs or circumstances to Coolo Care.
  • Acknowledge that Coolo Care is not a substitute for professional medical care or
    emergency services.

5.2. In the event of a medical emergency, Coolo Care will notify the responsible party but will not provide medical intervention.Coolo Care will notify staff immediately of the incident, if the emergency happens in home Coolo Care will dial 911 and wait for emergency services to arrive.

6. Limitations of Liability

6.1. Coolo Care is not liable for:

  • Medical issues or emergencies occurring between visits.
  • Errors or omissions in provided information by the responsible party.
  • Any medical, financial, or legal decisions made based on the documentation provided by Coolo Care.

6.2. By using Coolo Care services, the responsible party acknowledges that the services are non-medical and observational in nature.

7. Termination of Services

7.1. Either party may terminate services with a 14-day written notice. A full refund of undelivered serevices will be provided.

7.2. Coolo Care reserves the right to terminate services immediately if:

  • Documentation provided during intake is found to be fraudulent.
  • There are breaches of the Terms of Service.
  • Coolo Care reserves the right to change their Terms Of Service at any time. Clients will be notified via email of any changes.

8. Acknowledgment and Agreement

By signing below, you acknowledge and agree to the terms and conditions outlined above. You confirm that you are the guardian, POA, or responsible party for the individual receiving services and have provided accurate documentation during the intake process.
